Registrations will be accepted for all age groups (4 – 18 yrs olds)

T-ball – Farm      $115.00

Minors – Big League       $125.00

You must provide an ORIGINAL Birth Certificate; abstract Birth Certificates are not acceptable. 

You must also provide three (3) proofs of residency,
dated from September 1, 2011 – January 31, 2012.

proofs of residency

Valid Driver’s License
Voter’s registration
School records
Welfare/child care records
Federal records
State records
Local (municipal) records
Support payment records
Homeowner or tenant records
Utility bill (i.e. gas, electric, water/sewer, phone, mobile phone, heating, waste disposal) – only one from this category
Financial (loan, credit, investments, etc.)
Insurance documents
Medical records
Military records
Internet, cable, or satellite records
Vehicle records
Employment records

All proofs must show the same address and cannot show a P.O. Box.  Please make sure school records indicate your residence address, otherwise, we cannot accept it.

Additional information needed: Date of your child’s last Tetanus Booster is also needed.  Family physician, address, hospital preference, Health Insurance and Group number; Dentist, address, Dental Insurance and Group Number.

NOTE : No registration will be accepted unless all the above information is provided.
